This form is intended to be used by NABC trainers in requesting permission to hold an evaluation session. if you have not participated as a trainer in an evaluation session, or if your site has never hosted a session, we encourage you to contact the appropriate taxonomic committee chair person for guidance in understanding the requirements to host a session.

Include site, nearest city, state/province, and country.
Include dates, whether the workshop is a required part of the evaluation process, and whether the workshop is introductory or advanced.
Include the average number of birds captured (including recaptures), as well as the range. Taxa whose evaluations do not require live birds in hand should just respond N/A.
